MemoryLane Care Services Launches Care Transitions Program

MemoryLane Care Services introduces the Care Transitions Intervention program to help
individuals and families navigate the often-stressful return home after a hospital stay. Designed for people living with dementia or developmental disabilities and their caregivers, the program provides personalized guidance to ensure a safer, smoother recovery at home. Participants are paired with a dedicated Transitions Coach who visits in the hospital and at home, with follow-up phone support to help manage medications, recognize warning symptoms, coordinate follow-up appointments and keep health information organized.
